Preparing a Lasting Power of Attorney (LPA) allows you (the Doner) to appoint a person (the Donee) to help you make decisions on your behalf in the event you lose mental capacity.
Who Can Use This Service?
To make an LPA, you must be at least 21 years old and:​
​
• have the mental capacity and ability to make your own decisions
• not be an undischarged bankrupt if you wish to make an LPA for property and affairs matters
• you wish to appoint no more than 2 Donees
​
At SingMedical, we can only help you with Form 1. If you wish to appoint more than 2 Donees, appoint more than 1 Replacement Donee, grant your Donees specific or customised powers, then you will need to complete Form 2 with the help of a lawyer.

To use this service, you'll need a Singpass account and the Singpass app on your mobile phone.

You’ll also need the personal details of who you want to grant powers to (the Donees), including their:
• full name
• NRIC or FIN
• local mobile number

Once you have completed the above, it’s time to pay us a visit so we may certify your application.

After checking that you are of sound mind and made your plans of your own free will, the doctor will guide you through the final steps in OPGP portal using steps shown in the video above.
